Anyway for a pop up box to enter date into?

So when you press Take it pops a box up that asks you to enter the current date... maybe even from a pull down list?

Wish I had programmer skills or I would gladly help with this.

I have thought of this but I didn't want to force people to enter a date. I can still use the data without the date, but the date just gives me better data.

Make it an option?
